About This Service

Everyone Deserves to Be at Home at the End of Their Life

When someone we love is approaching the end of their life, the question that matters most is often the simplest one: where do they want to be? For most people, the answer is home. Carlisle Care exists to honour that wish — providing the compassionate, professional care that makes it possible for people to spend their final weeks and days in the place that is most meaningful to them.

Our palliative and end-of-life care service is built around the individual and their family — not around a clinical checklist. We focus on comfort, dignity, and presence. We work quietly and carefully alongside the person's wishes, their loved ones, and their existing clinical teams — ensuring that nothing important is missed and that no one is left to carry this alone.

We understand that arranging palliative care — or even enquiring about it — is one of the most difficult steps a family can take. Our team approaches every conversation with gentleness and patience. There is no script, no pressure, and no checklist. There is only a team that genuinely cares, ready to listen.

“We are not here to manage the end of life. We are here to support the person through it — and to stand alongside the people who love them.”

How We Work

Our Approach to Palliative and End-of-Life Care

Every aspect of palliative care at Carlisle Care is guided by one principle: the person and their wishes come first — always. We do not impose a structure or a timeline. We follow the individual, listen to their family, and coordinate with their clinical team to provide care that is as quiet or as present as the person needs it to be.

Our carers who work in palliative care are selected and trained specifically for this role. They are not simply capable practitioners — they are people of exceptional compassion and steadiness, able to be present in the most profound moments of a person's life without intruding on them.

  • Comfort Over ProcedureEvery decision is led by what brings comfort to the person — not by what is clinically routine. We follow their lead, their preferences, and the guidance of their palliative team.

  • Dignity in Every InteractionPersonal care, medication, repositioning, hygiene — every physical task is carried out with the most gentle, respectful approach. The person is never hurried or made to feel like a task.

  • A Calm, Consistent PresenceOur carers are trained to be steady — to bring calm into the room, not anxiety. A familiar face the person and family have come to trust.

  • Family Supported TooWe actively support the family — communicating clearly, answering questions honestly, and making sure no one feels alone in what they are carrying.

  • Coordinated With the Clinical TeamWe work alongside hospice teams, Macmillan nurses, district nurses, and GPs — never in isolation. Care is joined up, safe, and consistent.

For the Family

We Are Here for the Whole Family — Not Just the Person in Our Care

The end of a loved one's life is one of the most profound and exhausting experiences a family can go through. The emotional weight of being present, of managing practical arrangements, of watching someone you love become more unwell — it is immense. And it is often carried quietly, without enough support.

Carlisle Care's palliative care team is trained to support the family too — not as an afterthought, but as a central part of our role. We communicate honestly and clearly. We answer questions as gently as possible. We encourage families to rest when we are present. And we make sure that no one feels left to manage this entirely alone.

💬

Honest, Gentle Communication

We keep family members informed — with regular, clear updates delivered with sensitivity and care. Questions are always welcome, and we answer them as honestly as we can.

🤝

Practical and Emotional Support

We take on the practical tasks that allow families to be present with their loved one — and we recognise when a family member needs a moment of quiet acknowledgement themselves.

🌿

After the Loss

When the time comes, we signpost families to bereavement support resources and remain available for any practical questions about next steps — no one needs to navigate this alone.

Working Together

Working Alongside Your Clinical Team

Palliative care works best when everyone involved is coordinated. We actively communicate with the clinical teams already supporting your loved one.

🏥

Macmillan & Hospice Teams

We work closely with Macmillan nurses and hospice at home teams — sharing information, attending reviews where appropriate, and ensuring our care complements their clinical guidance without duplication or conflict.

👨‍⚕️

GPs and District Nurses

We maintain regular communication with the individual's GP and any district nurses involved in their care — reporting changes in condition promptly and following clinical advice on symptom management and medication.

👥

Social Workers and Discharge Teams

For those transitioning from hospital to home for end-of-life care, we coordinate with hospital discharge teams and social workers — ensuring care is in place before or immediately upon arrival home.

“Dad spent his final three weeks at home, exactly as he always said he wanted. Carlisle Care made that possible. The carer who came to him was remarkable — gentle, patient, and genuinely kind. She became part of those last weeks in the most beautiful way. As a family, we are so grateful.”
