We work closely with Macmillan nurses and hospice at home teams — sharing information, attending reviews where appropriate, and ensuring our care complements their clinical guidance without duplication or conflict.

We maintain regular communication with the individual's GP and any district nurses involved in their care — reporting changes in condition promptly and following clinical advice on symptom management and medication.

For those transitioning from hospital to home for end-of-life care, we coordinate with hospital discharge teams and social workers — ensuring care is in place before or immediately upon arrival home.
